PERSONNEL FILE AND MEDICAL RECORDS
The company maintains a personnel file for each employee. All information relating to employment is contained in the file, including information such as your employment application, resume (if submitted), performance evaluations, etc. This information is kept confidential. It is important that you keep the company current on information effecting your employment, including personal information. For example, the company must be kept apprised of changes in your name, address, telephone number, marital status, dependents, etc. The company must know much of this information because of your employee benefits. Please notify [specify company official or department (e.g. the Human Resources Department)] of any changes in such information. Employee medical information is not maintained in your personnel file. It is kept in a separate, confidential file.
