PERFORMANCE EVALUATION

At the end of your probationary period, your work performance will be reviewed to assess your strengths, weaknesses, areas needing improvement and employment status. Thereafter, your job performance will be reviewed by your supervisor [specify period (e.g., annually)] in [specify month]. Your supervisor will meet with you to discuss the evaluation and give you the opportunity to comment on it in writing. Performance evaluations become part of your permanent personnel file.
