SHORT TERM DISABILITY SAMPLE
Each employee will be automatically enrolled in a Short Term Disability benefit plan upon the first day of the month following 90 calendar days of employment. Only those part time employees who work less than 20 hours will not be eligible.
If you are disabled due to a non-work related incident, and you are out of work with a doctor's note for at least 7 days, you will receive up to 2/3 of your normal average pay for a period of up to three months. You will need to complete these forms for enrollment and they will be forwarded to you at the appropriate time.
